CLASSIC POINT- Transition Living Home:

This program offers unique transitional services to help individuals gain skills to support future semi-independent or independent living. This setting supports individual comfort, independence and preferences. The individuals have comfortable living area for private visits with family and friends.

This residential transitional program targets individuals that appear to be behaviorally stable and /or have the potential for learning skills or compensatory strategies for more independent living. The program offers individualized opportunities for skill building through community interaction and engagement in social, productive and recreational endeavors.

The individuals living at this site are employed in an integrated settings and/or active outside of this residential setting. If the individual would like to work, there is an activity that ensures the option is pursued. More importantly, the individual has unrestricted access in this setting. All individuals have full access to the community.

Program services are driven by consumers choices and self-determination. The individual chooses and controls a schedule that meets a schedule that meets his/her wishes in accordance with a person-centered plan. Individual choices are incorporated into services and supports received in this setting.
